Low

Low risk on this score. It does not exclude an acute coronary syndrome. Serial troponin and ECG still apply.

14-day risk of death, MI or urgent revascularization
4.7%

The formula

  • One point for each of the seven criteria. Range 0 to 7.

Worked example

A 70-year-old on aspirin with ST depression and a raised troponin: 1 + 1 + 1 + 1 = 4, intermediate risk

When to use it

  • Risk-stratifying a patient with unstable angina or NSTEMI on arrival

And when not to

  • It is validated for UA/NSTEMI. It is not the score for STEMI, which has its own.
  • A low score is not a discharge decision. It stratifies risk within a diagnosis, it does not make one.
