The formula

  • MAP = diastolic + (systolic − diastolic) ÷ 3

Worked example

120/80 → 80 + (120 − 80) ÷ 3 = 80 + 13.3 = 93 mmHg

When to use it

  • Assessing perfusion in shock or sepsis
  • Titrating vasopressors against a written target

And when not to

  • The ⅓ approximation assumes a normal heart rate. At high rates diastole shortens and the estimate drifts upward of the true mean.
  • An arterial line measures MAP directly. Where one is in place, use it rather than this.
