APGAR score
The APGAR score describes a newborn’s condition at one and five minutes across five signs, each scored 0 to 2. It records how the baby is, and how the baby responds to whatever is being done. It does not predict outcome on its own.

The formula
- Five signs, each 0 to 2: appearance, pulse, grimace, activity, respiration. Range 0 to 10.
Worked example
Pink body with blue hands (1), heart rate 120 (2), grimace (1), some flexion (1), crying (2) = 7
When to use it
- At one and five minutes after birth, and every five minutes to twenty if the score stays low
And when not to
- It never delays resuscitation. Resuscitate first; score afterwards.
- A single APGAR does not predict long-term outcome and is not evidence of asphyxia.
- Prematurity lowers tone and reflex scores independently of any illness.
