The formula
- HOMA-IR = (fasting insulin µU/mL × fasting glucose mg/dL) ÷ 405
- Using mmol/L: HOMA-IR = (insulin × glucose mmol/L) ÷ 22.5
Worked example
Insulin 15 µU/mL with glucose 100 mg/dL → (15 × 100) ÷ 405 = 3.7
When to use it
- Assessing suspected insulin resistance: polycystic ovary syndrome, metabolic syndrome, fatty liver
And when not to
- It is not valid in a person on insulin, or in type 1 diabetes.
- Insulin assays are not standardized between laboratories, so values are not comparable across sites.
- It is an index, not a diagnosis. Nothing is treated on the basis of a HOMA-IR alone.
