The formula
- CKD-EPI 2021: eGFR = 142 × min(Scr/κ, 1)^α × max(Scr/κ, 1)^−1.200 × 0.9938^age × 1.012 if female
- κ = 0.7 (female) or 0.9 (male); α = −0.241 (female) or −0.302 (male)
- MDRD: eGFR = 175 × Scr^−1.154 × age^−0.203 × 0.742 if female
Worked example
A 60-year-old man with creatinine 1.2 mg/dL, CKD-EPI 2021 → approximately 68 mL/min/1.73 m²
When to use it
- Staging chronic kidney disease
- Tracking kidney function over time
And when not to
- eGFR is indexed to body surface area; creatinine clearance is not. Do not use one where a drug label asks for the other.
- It is not valid in acute kidney injury, in pregnancy, or at the extremes of muscle mass.
- Report which equation you used, because a laboratory switching from MDRD to CKD-EPI changes the number without the patient changing.

Cockcroft-Gault estimates creatinine clearance from age, weight, sex and serum creatinine. It is the equation most drug labels are written against, which is why it survives alongside eGFR.
Fractional excretion of sodium (FENa)
FENa is the proportion of filtered sodium the kidney excretes, and it separates pre-renal acute kidney injury from intrinsic tubular injury. Under 1% suggests the kidney is holding on to sodium; over 2% suggests it cannot.
